WHAT TO EXPECT AT YOUR FIRST VISIT
Let’s get to know each other.
We always welcome new patients and your first chiropractic care visit will consist of an initial consultation and examination. Your concerns will be discussed with the doctor to understand your symptoms and how they are affecting your abilities. A thorough picture helps us to provide you with the best possible care.
We will perform a series of functional movement assessments, muscle length assessments, joint range of motion assessments, orthopedic tests, neurologic and functional examinations to determine the cause of your particular problem.
Your doctor will explain the findings and let you know what may be contributing to the problem and discuss with you your expected course of treatment.
Treatments may include: joint mobilizations, specific stretching and stabilization exercises, decompression therapy, electrical muscle stimulation, Graston Technique, Active Release Technique (ART), heat, ice, cold laser, and/or kinesiotaping. We have a wide variety of tools and modalities here at the clinic and are able to provide a multi-faceted approach to your treatment.
Your doctor will discuss short term and long term goals with you and give you realistic expectations regarding your treatment plan.
Please plan on 2 hours for your initial visit, possibly more, depending on the complexity and severity of your case.
WHAT TO BRING
Our office will send you a link to the client portal to complete all new patient forms prior to your scheduled appointment
Please arrive 15 minutes early to ensure all forms, insurance, payment are processed and completed so we can provide you your entire scheduled time efficiently
Please bring to your scheduled appointment:
Current insurance card
Any medical information including: X-ray/MRI imaging and reports
Wear or bring COMFORTABLE LOOSE ATHLETIC CLOTHING for easy access to areas of injury for treatment (athletic shorts and loose top like a tee shirt or tank top - not fitted)
CANCELLATION POLICY
Please call us 24 hours before your scheduled appointment to notify us of any changes or cancellations at 408-288-8120
If prior notification is not given, you will be charged $50 for the missed appointment
INSURANCE
Please make sure to bring your current insurance card with you to your first appointment.
We are only IN-network with Valley Health.
You can still be seen in our office even if you have a PPO insurance that we are not contracted with.
We accept the following insurance below as ‘Out of Network’, and some others not listed:

UMR PPO
United Health Care PPO
Meritain Health PPO
We do not participate with American Specialty Health, Kaiser, and Medicare.
In addition, we do not take Workman’s Compensation or Personal Injury cases.
